Job summary

BrightStar Care of Fort Wayne is seeking a Direct Support Professional to provide one-on-one, in-home care in Lafayette, IN. Pay ranges from $15.00 to $18.00 per hour, with both full-time and part-time options and consistent 1st or 2nd shift opportunities.

The DSP will assist with ADLs, support independence, and ensure client safety, comfort, and dignity while maintaining HIPAA-compliant documentation and reporting to the nurse case manager.

Qualifications

• High school diploma or GED required.
• At least 1 year of professional DSP, caregiving, or home care experience.
• Current CPR certification required.
• Background check and drug screen required.
• Valid driver's license, auto insurance, and reliable transportation.
• Ability to lift up to 50 pounds and perform physical duties.

Responsibilities

• Assist with activities of daily living (ADLs) such as bathing, grooming, dressing, and personal hygiene.
• Support clients with daily routines while promoting independence.
• Provide companionship and emotional support.
• Assist with meal prep, light housekeeping, errands, and community activities.
• Support mobility, transfers, and safety needs as per care plan.
• Follow individualized care plans and document care accurately.
• Observe and report changes in client condition to nurse case manager.

This is an excellent opportunity for an experienced DSP, Caregiver, Home Health Aide (HHA), or Personal Care Assistant (PCA) seeking consistent hours, supportive leadership, and the opportunity to help individuals remain safe, comfortable, and independent.

Qualifications

• High school diploma or GED
• At least one year of professional DSP, caregiving, or home care experience
• Current CPR certification
• Current physical examination and negative TB test completed within the past 12 months
• Valid driver’s license, auto insurance, and reliable transportation
• Ability to lift up to 50 pounds and perform the physical duties of the position
• Ability to pass a background check and drug screen
