Job summary

The role supports goals from each Centered Plan, assisting with daily activities, transportation, and community integration in home and community settings.

You’ll wear many hats—advocate, coach, and care team member—while fostering independence, dignity, and choice. Requires empathy, reliability, and strong communication skills.

Qualifications

• High School Diploma or GED preferred.
• Must be at least 18 years old.
• Valid driver’s license may be required for transportation duties.
• Ability to work collaboratively with care teams and individuals served.

Responsibilities

• Promote healthy living and prevent illness and accidents in the community and home.
• Accompany individuals to work, activities, and community outings.
• Teach independent living skills and support decision-making in daily life.
• Provide transportation to appointments and assist with personal care tasks as needed.
• Document care provided and protect confidentiality.
