Job summary

Monarch Healthcare in North Carolina is seeking a PRN direct care worker to support individuals’ daily living and community participation in a residential setting. Sleepover shifts are required, with coverage on an as-needed basis and occasional late-night support.

You will assist with daily activities, safety, and advocacy, maintaining documentation and collaborating with the team. A valid driver’s license and state compliance are required.

Qualifications

• High School Diploma is required.
• Valid USA Driver's License.
• Experience with individuals with mental health or developmental disabilities is preferred but not required.

Responsibilities

• Assess people for hopes and goals and provide supports to help them achieve.
• Help individuals develop relationships in the community and with natural supports.
• Assist with daily living activities and community participation.
• Protect and promote the rights of people supported; support advocacy efforts.
• Provide emotional, physical, and medical support as needed.
• Document progress, goals, incidents, timesheets, and other required items.
• Complete notes and logs; attend meetings and trainings; maintain certifications.
