Qualifications

• At least 18 years of age.
• High School Diploma, GED, or equivalent.
• Ability to pass a background check with the MN DHS.
• General computer skills and knowledge.
• Reliable transportation insured in Minnesota.
• Proof of insurance and current driver’s license.
• Clear driving record (no DUI/DWI in past 5 years).
• Fluency in reading, speaking, and writing in English.
• Dynamic communication skills.
• Working cell phone with internet access.

Responsibilities

• Assist clients in behavioral and personal care services.
• Assist clients with activities of daily living per care plans.
• Ac company clients on recreation and leisure activities.
• Implement clients' plans as written.
• Increase clients' independence and involvement in their community.
• Understand and implement Behavior Support Plans and ISPs.
• Track and chart data daily in line with company expectations.
• Attend and engage in team meetings as needed.
