Responsibilities include ensuring health, safety, and progress toward independence, documenting client status, and administering behavior support plans. This role requires PA certifications and CPR/health screening compliance.

Qualifications

• Must read and write proficiently in English.
• Successful completion of the Direct Care Worker Competency evaluation.
• Current CPR Card.
• Current Health Screening per state regulations.
• Provide proof of PA nursing license or PA CNA or Home Health Aide training or equivalent.

Responsibilities

• Ensure client’s Health, Wellness and Safety.
• Encourage independent living skills related to learning and daily living.
• Crisis prevention and intervention.
• Promote advocacy and individual rights.
• Maintain detailed and compliant documentation on client status and progress.
• Implement behavior support plan strategies appropriate to the individual.
• Report suspected abuse, neglect, and exploitation immediately.
• Utilize prompting, instruction, modeling, and reinforcement for person-centered services.
• Promote independence in daily living, work, learning and leisure.
• Provide transportation based on client needs.
• Complete required annual trainings to maintain certifications.
