The role emphasizes health, safety, and promoting independence under person-centered plans. Responsibilities include crisis prevention, documentation, adherence to behavior support plans, and facilitating learning of daily living skills, with required CPR and state screenings to maintain credentials.

Qualifications

• Ability to read and write proficiently in English.
• Successful completion of the Direct Care Worker Competency evaluation process.
• Current CPR card.
• Current Health Screening as per state regulations.
• Proof of Pennsylvania nurse license or PA certified nurse aide OR Home Health Aide certification or equivalent per state requirements.

Responsibilities

• Ensure client’s Health, Wellness and Safety.
• Encourage individual goals related to learning and independent living skills.
• Crisis prevention and intervention.
• Promote advocacy and individual rights.
• Maintain detailed documentation on client status, interactions and behaviors.
• Implement behavior support plans safely and appropriately.
• Report suspected abuse, neglect or exploitation immediately.
• Use prompting, instruction, modeling and reinforcement in person-centered services.
• Promote independence in activities of daily living and community participation.
• Provide transportation based on client needs.
• Complete required annual trainings to maintain certifications.
