Job summary

Assured Wellness LLC is seeking a Direct Support Professional (DSP) in Minnesota to assist adults with mental health diagnoses in licensed ALF/ICS settings. You will follow MDH, ALF, and 245D guidelines to support daily living, health, safety, and community participation.

You will provide person-centered care, document services, and collaborate with the team to implement individualized plans while upholding privacy and dignity for residents.

Qualifications

• Must be at least 18 years old.
• High school diploma or GED required.
• Must pass a background check and any required health screenings.
• CPR/First Aid certification or willingness to obtain upon hire.

Responsibilities

• Assist with Activities of Daily Living (ADLs).
• Support mental and behavioral health needs.
• Use a person-centered, trauma-informed approach to support emotional regulation, coping skills, and recovery-based strategies.
• Provide medication reminders or administer medications.
• Implement individualized service plans and behavior support strategies in alignment with resident’s coordinated services and treatment plan.
• Promote community integration by supporting residents in accessing employment, volunteering, education, and social activities.
• Accurately complete daily documentation on all services provided, progress notes, changes in behavior or functioning, and incident reports as required by 144G, 245I, 245D, and organizational policy.
• Ensure compliance with Assisted Living Bill of Rights, HIPAA, Vulnerable Adults Act, and Data Practices.
• Uphold everyone’s rights, dignity, privacy, and cultural preferences.

As a Direct Support Professional (DSP), you will play a vital role in assisting adults with mental health diagnoses residing in a licensed Assisted Living Facility (ALF), and/or in our Integrated Community Support (ICS) multi-unit buildings. The DSP will provide person-centered services that follow MDH, ALF, and 245D guidelines to support individuals with household management, health and safety, community participation, and adaptive skills.

Key Responsibilities

• Assist with Activities of Daily Living (ADLs)
• Support mental and behavioral health needs

• Shifts Day 7am-3pm Evening 3pm-11pm Night 11pm-7am Assured Wellness LLC facilities are supported 24/7 to ensure ultimate care and safety for our residents.

Requirements

• Must be at least 18 years of age
• High school diploma or GED required
• Valid driver’s license and reliable transportation
• Must pass a background check and any required health screenings
• Knowledge basic computer skills for documentation and communication
• Strong interpersonal skills, professionalism, and ability to work independently
• Ability to work flexible shifts, including nights, weekends, and holidays
• CPR/First Aid certification (or willingness to obtain upon hire)
