Job summary

New Horizons in Watertown, SD is seeking a Direct Support Coordinator to lead and support a team providing services to individuals with disabilities. You will advocate for rights, monitor plans, and ensure high-quality, person-centered supports.

The role requires strong leadership, excellent communication, and the ability to work independently while collaborating with a team. CPR/First Aid and medication certification are often required.

Qualifications

• Experience working with people with disabilities preferred.
• Supervisory experience preferred.
• CPR, First Aid and Medication Certification training may be required.

Responsibilities

• Support people in achieving their goals and desired outcomes.
• Advocate for individuals and ensure their rights and needs are respected.
• Develop and monitor Individual Support Plans.
• Provide leadership, supervision, and support to Direct Support Professionals.
• Ensure staff follow individual plans, agency policies, and state regulations.
• Monitor staffing, schedules, training, documentation, and service quality.
• Communicate and collaborate with individuals, families, guardians, professionals, and team members.
• Maintain accurate records and complete required documentation on time.
