Job summary

This role includes active, awake positions across locations, completing required documentation, assisting with daily routines and recreational activities, and ensuring safety and compassion in care. Applicants must meet education and certification requirements and be able to work flexible

Qualifications

• High school Diploma/GED required.
• Valid Driver's license with a clean MVR to drive agency vehicles.
• Must obtain and maintain medication certification.
• Ability to write legibly.
• Ability to communicate effectively with other disciplines and individuals served.
• Ability to perform physical interventions.

Responsibilities

• Maintain an environment of care that is safe and therapeutic.
• Provide direct care to individuals per agency policy.
• Implement supervision to daily schedules and life skills activities.
• Complete required documentation in compliance with Agency, Medicaid, and OPWDD standards.
• Dispense medication per agency policy.
• Transport individuals safely as assigned per level of supervision required.
• Assist individuals with tasks as designated by individual treatment plan.
• Participate in the treatment team or recommendation process.
• Assist in personal care of individuals.
• Plan and develop recreational activities.
• Orient new admissions to the agency.
• Adhere to DSP Code of Ethics.
• Meet OPWDD Core Competency standards.
• Complete required trainings and stay current with trainings.
• Provide a monthly schedule by the 15th to the supervisor.
• Work a minimum of 20 hours per week at assigned locations.
• Adhere to program staff requirements.
• Complete assignments as directed by supervisor.
