Claratel Behavioral Health is hiring a Direct Service Professional in Georgia to support individuals with intellectual and developmental disabilities across home, work, and community settings. You will help with daily living activities, health maintenance, and social skills development while following care plans and documenting progress.

Qualifications

• High school diploma or GED is required.
• Valid Georgia Driver’s License with an acceptable MVR.
• Ability to lift 50 lbs unassisted and assist mobility.
• Regular bending, squatting, standing, and moving throughout your shift.
• Strong communication skills, basic computer literacy, and the ability to manage conflict effectively.

Responsibilities

• Empower daily living by assisting with cooking, cleaning, hygiene, health maintenance, and medication self-administration.
• Foster independence through social skills, community involvement, ISP/BSP input and strategies.
• Ensure safety by following care plans and reporting risks.
• Be a mentor and role model, coaching individuals through challenges and relationships.
• Prepare documentation: progress notes, incident reports, and logs.
• Provide transportation to appointments and events.
• Collaborate with teammates and families for best support and professional growth.
• Participate in ongoing agency improvement initiatives.
• Assist with daily household routines (laundry, meal planning, groceries, chores).
• Follow medication administration procedures (SSAM/Proxy Caregiver).
• Maintain clear communication logs during shift transitions.
