Job summary

Hacienda HealthCare in Phoenix, AZ is hiring Direct Service Professionals for the Day Treatment for Adults (DTA) program. The role supports residents under the Program Coordinator's supervision and includes driving duties for some shifts.

Responsibilities involve ensuring residents' safety, personal care, meal support, behavior and activity engagement, and accurate documentation of ISP objectives, with a safety-sensitive classification for this position.

Qualifications

• Must be at least 18 years of age.
• High School diploma or equivalent required.
• Must have or be capable of passing and maintaining Article 9 and Prevention and Support within 90 days of hire.
• Must have or be capable of passing CPR / 1st Aid within 30 days of hire.
• Must have or be capable of obtaining and maintaining a Class 1 Fingerprint Clearance Card.
• Background check and drug screen required.

Responsibilities

• Provide safety, emotional, and physical support for residents.
• Assist with daily living activities and meal preparation.
• Position and reposition residents as needed; ensure comfort.
• Document ISP objectives daily and maintain required records.
• Assist residents with showers and laundry; monitor cleanliness.
• Drive residents to appointments and activities; maintain driving records.
