Job description

Direct Service Professional at Hacienda Healthcare in Phoenix, Arizona (1402 E South Mountain Ave, 85042). Full‑time, Monday through Friday. Shift options: 8:00 am–4:00 pm (non‑driver), 9:00 am–5:00 pm (driver), 9:00 am–5:30 pm (driver). Hiring bonus available. Reports to Program Coordinator. This role is classified as safety sensitive.

Responsible for Residents

• Provide for the safety, emotional and physical needs of residents.
• Ensure residents are clean, well‑groomed and dressed appropriately for the season and environmental conditions.
• Position and reposition every hour while awake and every two hours while asleep, and as needed.
• Ensure ISP objectives and data are implemented and documented according to the residents’ ISP every day.
• Help residents cook meals and eat as needed.
• Ensure residents receive emotional and physical stimulation to the best of their ability.
• Assist with showering residents to ensure a complete shower is given.
• Assist with resident laundry to ensure it is clean.
• Pass medications in non‑medical group homes safely.

Driving Responsibilities

• Perform daily maintenance checks of vehicles (e.g., checking turn signals, cleanliness, fuel).
• Keep signed documents (Med Appointment Forms) and return them to Manager upon completion of appointments.
• Escort and drive residents to appointments and activities as scheduled.
• Maintain a clean driving record.
• Report any changes to your driving record immediately.

Responsible for the Home

• Clean the home, including but not limited to all areas and resident bedrooms.
• Ensure required documentation (incident reports, HT daily duties, etc.) is completed before the end of shift.
• Perform duties following safety standards, including use of Personal Protective Equipment (PPE) such as gloves.
• Attend training sessions required by the company (e.g., house meetings).

Responsibility to the Company

• Be flexible with hours and location to assist with staff coverage if needed.
• Ensure compliance with:
  • Article 9
  • Prevention and Support
  • Annual TB skin test
  • Fingerprint Clearance Card
  • CPR & First Aid
  • Annual flu vaccine (or always wearing a mask if refusing flu vaccine)
  • Valid driver’s license in good standing
• Adhere to and follow directives, policies, and procedures handed down by the company.
• Ensure required documentation (ISP outcomes, incident reports, HT daily duties, etc.) is completed before the end of shift.
