Job summary

Frontier Direct Care in Texas is hiring Family Nurse Practitioners and Physician Assistants to serve as the primary clinician for a defined patient panel. You’ll provide care across chronic disease management, preventive care, and acute visits with longer visit times in a lower-volume, high-touch model.

You’ll work under a collaborating physician, conduct visits in person and via secure text/video, and maintain documentation in the EHR.

Qualifications

• Active, unencumbered Texas licensure as a PA (NCCPA-certified) or APRN‑FNP.
• Minimum 2 years post-graduate experience in family practice or primary care.
• Experience managing patients across lifespan (pediatric to geriatric).
• Active, unrestricted DEA registration or ability to obtain before start.
• Current BLS certification or ability to obtain within 30 days.
• Proficiency with electronic medical records and scheduling software.

Responsibilities

• Provide longitudinal primary care across chronic disease, preventive care, and acute visits.
• Perform in-office procedures including I&Ds, suturing, skin biopsies, and health exams.
• Conduct visits in person, by phone, and secure video with 30–60 minute lengths.
• Diagnose and manage chronic conditions and coordinate care with Medical Assistant.
• Document visits and maintain patient records in the EHR.
• Maintain 24/7/365 on-call access via a secure line under supervision rules.
• Participate in weekly team meetings and 1:1s as requested.
