Job summary

Frontier Direct Care, LLC is hiring Family Nurse Practitioners or Physician Assistants to lead primary care for employer-sponsored patients. This full-scope role emphasizes continuity, preventive care, and chronic disease management with longer visits and lower daily volume.

You will manage chronic conditions, perform in-office procedures, and build lasting patient relationships under Texas supervision rules, with team collaboration and secure patient communication.

Qualifications

• Active Texas PA license (NCCPA) or APRN-FNP with board certification.
• Minimum 2 years in family practice or primary care.
• Experience managing patients across lifespan and in-office procedures.

Responsibilities

• Provide longitudinal primary care for chronic disease, preventive, and acute visits.
• Perform in-office procedures such as I&Ds, suturing, biopsies, and health exams.
• Manage patient panels with 30–60 minute visits and coordinate with Medical Assistant.
• Document visits in the EHR and maintain patient records.
