Job summary

NestVillage LLC is seeking a Direct Care Worker to support individuals with developmental disabilities in a group home. You will assist with daily living activities, monitor health, and follow care plans under licensed nursing supervision.

You will provide companionship, engage residents in activities, and help build life skills toward independence. This full-time role requires flexibility, weekends, and adherence to safety protocols.

Qualifications

  • High school diploma or GED required; relevant training or certification (CNA/HHA/DSP) preferred.
  • CPR/First Aid certification
  • Valid Fingerprint Clearance Card
  • Experience with individuals with developmental disabilities in residential/group home setting preferred.
  • Strong interpersonal skills with residents' trust and rapport.
  • Valid driver’s license with clean driving record and ability to transport residents.
  • Three professional references required.

Responsibilities

  • Assist residents with activities of daily living (ADLs) such as bathing, dressing, toileting, feeding, and mobility.
  • Monitor residents’ health, behavior, and well-being; promptly report concerns to supervisors.
  • Administer prescribed oral medications and provide basic health-related support per care plans.
  • Offer companionship and engagement in recreational, educational, and social activities.
  • Support residents in learning and practicing life skills to encourage independence.

Job Summary:

The Direct Care Worker supports individuals with developmental disabilities in a group home setting, ensuring their health, safety, and overall quality of life. This role provides hands-on assistance with personal care, daily living activities, and health-related needs under the direction of licensed nursing staff. Direct Care Workers foster a supportive, respectful environment that promotes independence, skill-building, and community participation while maintaining compliance with agency, state, and federal standards.

Key Responsibilities:
Resident Care & Support:
  • Assist residents with activities of daily living (ADLs) such as bathing, dressing, toileting, feeding, and mobility assistance.
  • Monitor residents’ health, behavior, and overall well-being; promptly report concerns to supervisors.
  • Administer prescribed oral medications and provide basic health-related support such as changing dressings or checking vital signs, following established care plans.
  • Offer companionship, conversation, and engagement in recreational, educational, and social activities.
  • Support residents in learning and practicing life skills that encourage independence.
Home Environment & Daily Operations:
  • Prepare and serve meals according to residents’ dietary needs and preferences.
  • Assist with household upkeep, including laundry, cleaning, and organization, to maintain a safe, comfortable living space.
  • Accompany residents to medical appointments, community outings, or recreational activities, providing transportation and support as needed.
Communication & Documentation:
  • Accurately document care provided, changes in condition, and daily activities in compliance with agency policies.
  • Collaborate with families, healthcare professionals, and team members to ensure coordinated, person-centered care.
  • Follow all safety protocols, care plans, and state licensing requirements for group home operations.

Schedule & Availability Requirements

This is a full-time position. Applicants must be available to work at least four (4) days per week, and availability must include at least one weekend day. Shifts range from 6 to 12 hours depending on agency needs, employee preference, and availability. Flexibility in scheduling is essential to ensure adequate coverage and continuity of care for residents.

Work Environment:

Group home setting with close interaction with residents and staff. This role requires physical activity including lifting, assisting with mobility, and responding to emergencies. Flexible scheduling is necessary, including evenings, weekends, and holidays.
