Direct Care Worker at 365 Health Services

Responsibilities:

1. Assist patients with personal care activities such as hygiene, mobility, and elimination as prescribed in the care plan.
2. Maintain a safe environment for patients.
3. Use equipment safely and properly (footstools, side rails, oxygen, etc.).
4. Practice universal infection control precautions in all contacts with patients.
5. Assist with home maintenance activities like light housekeeping and meal preparation as prescribed.
6. Monitor and record fluid and nutrition intake, vital signs, skin condition, and other body functions, reporting changes appropriately.
7. Practice safe transfer, ambulation, positioning, and range of motion techniques.
8. Identify the need for intervention by other team members and report to the RN or Director of Clinical Services.
9. Respect patient and family confidentiality and autonomy.
10. Document provided services and responses accurately and submit documentation timely.
11. Perform within the limits of training and experience.
12. Ensure compliance with laws, regulations, and agency policies.
13. Manage caseload efficiently, delivering cost-effective care.
14. Meet or exceed established service standards.
15. Conduct business professionally and ethically.
16. Use supplies and equipment effectively.
17. Complete at least 12 hours of in-service training annually.
18. Adhere to the agency’s Code of Conduct and policies, including confidentiality and HIPAA guidelines.

Job Details:

• Job Types: Full-time, Part-time, Temporary
• Salary: $10.00 - $15.00 per hour (Starting), Holiday Pay, OT Rate for hours >40/week
• Benefits: Dental, Health, Vision insurance, 401K, Flexible schedule
• Schedule: 8, 10, or 12-hour shifts; day, evening, night, overnight shifts; choose your hours; Monday to Friday, weekends
