Reliance Home Health Caregivers is seeking a Direct Support Professional (DSP) in Bolingbrook, IL for in-person shifts. The DSP will follow care plans, document care, and provide supportive services to consumers per agency policies.

Requirements include a High School Diploma and at least 1 year of related experience; valid Illinois driver's license and access to a vehicle are preferred. Weekend/evening availability may be required.

Responsibilities

• Provide daily care: bathing, dressing, grooming; assist with eating and elimination; support transferring and mobility.
• Follow Individual Support Plan and document care accurately and timely.
