Reliance Home Health Caregivers in Illinois is seeking a Direct Support Professional (DSP) to provide compassionate in-home care under the CILA Homes program. You will assist with daily living activities, follow individualized care plans, and document changes in patient condition.

Requirements include a high school diploma and at least 1 year of direct care experience; experience with developmental disabilities is preferred. The role is in-person with flexible scheduling and benefits.

Qualifications

• High school diploma required.
• At least 1 year of related direct care experience.
• Experience with developmental disabilities preferred.
• Ability to follow care plans and document care.

Responsibilities

• Provide daily consumer care including bathing, dressing, eating and mobility.
• Collect data and report changes to the Program Coordinator.
• Document findings accurately and maintain confidentiality.
• Follow infection control practices and safety procedures.
• Travel between consumer sites as needed.
