Job summary

Altum Care Homes in Southfield, MI is seeking a compassionate Direct Care Professional to join our team. This role involves providing hands-on care, assisting with daily activities, and supporting residents in a home-like setting.

You will work closely with supervisors and healthcare professionals to follow individualized care plans, document progress, and ensure resident safety and comfort. A high school diploma or GED is required, CPR/First Aid certification preferred, and background checks

Responsibilities:

• Provide hands-on personal care and assistance with activities of daily living (ADLs) including bathing, grooming, and dressing
• Support residents with mobility, transfers, and ambulation as needed
• Assist with meal preparation, feeding, and ensuring proper nutrition
• Implement individualized care plans and document resident progress accurately
• Administer medications as directed and in accordance with care guidelines
• Foster a safe, supportive, and positive home environment for all residents
• Communicate effectively with supervisors, family members, and healthcare professionals regarding resident well-being

Requirements:

• High school diploma or GED required
• Prior experience in direct care, personal care, or a related caregiving role preferred
• CPR and First Aid certification (or willingness to obtain)
• Ability to pass a background check and drug screening
• Strong interpersonal and communication skills with a patient, empathetic demeanor
• Reliable, punctual, and committed to consistent attendance
• Ability to lift, transfer, and assist residents with physical needs
