Location & Schedule

• Location: Los Angeles, CA (Koreatown, Aroma Building)
• Work schedule: Full-time, Monday through Friday, 9:00 AM – 6:00 PM
• Compensation: $23 – $28 per hour, depending on qualifications, experience, and position level

Responsibilities

• Monitor health data for patients with chronic conditions such as hypertension and diabetes and follow up on routine alerts.
• Monitor patient-generated health data, conduct patient outreach and support calls, and maintain accurate documentation in compliance with HIPAA requirements.
• Coordinate and communicate effectively with clinic staff and healthcare providers.
• Visit prospective clinics to introduce RPM and CCM programs, develop collaborative relationships, and identify new business opportunities.

Requirements

• Minimum one (1) year of clinic experience or care coordination experience (MA certification)
• Strong communication and documentation skills
• Computer proficiency, including Email, Microsoft Word, and Excel
• Bilingual in Korean and English
• Authorized to work in the United States
