Job summary

Snowline Hospice of El Dorado is seeking a Contract Dietitian to provide expert medical nutrition therapy for patients in hospice and palliative care. You will conduct nutrition assessments, develop care plans, educate patients and families, and collaborate with an interdisciplinary team to support comfort and quality of life.

This role requires RD/RDN credentials and California licensure, with at least two years of clinical nutrition experience.

Qualifications

• RD/RDN credential required.
• California licensure active.
• Minimum 2 years clinical nutrition experience; hospice/palliative care preferred.
• Strong interpersonal and communication skills.
• Ability to work independently with a flexible schedule.

Responsibilities

• Conduct comprehensive nutrition assessments and develop care plans for hospice/palliative patients.
• Provide dietary counseling and education to patients, caregivers, and staff.
• Collaborate with the interdisciplinary care team to align nutrition with care goals.
• Monitor patient progress and adjust nutrition recommendations as needed.
• Ensure compliance with regulations and best practices in medical nutrition therapy.
• Document patient interactions and care plans in EMR in a timely manner.
• Stay current with evidence-based guidelines in hospice nutrition.

Key Responsibilities

• Provide dietary counseling and education to patients, caregivers, and staff to optimize nutritional well-being.
• Collaborate with the interdisciplinary care team to align nutritional plans with overall patient care goals.
• Monitor patient progress and adjust nutrition recommendations as needed.
• Ensure compliance with applicable regulations, policies, and best practices in medical nutrition therapy.
• Document patient interactions and care plans in electronic medical records in a timely manner.
• Stay current with best practices and evidence-based guidelines in nutrition and hospice care

Requirements

• Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential required.
• Current licensure in the state of California.
• Minimum of two years of experience in clinical nutrition, preferably in hospice, palliative care, or home health settings.
• Strong interpersonal and communication skills to effectively engage with patients, caregivers, and healthcare teams.
• Ability to work independently and manage a flexible schedule based on patient needs.
• Knowledge of nutrition-related concerns in end-of-life care, including symptom management and palliative nutrition strategies.
