Snowline Health is seeking a Contract Dietitian to provide expert medical nutrition therapy for hospice and palliative patients. You will perform nutrition assessments, formulate care plans, and collaborate with a multidisciplinary team to support compassionate, patient-centered care.

The role requires an RD/RDN with California licensure and 2+ years of clinical nutrition experience, preferably in hospice settings.

• RD/RDN credential required.
• Current California licensure.
• Minimum of two years of clinical nutrition experience, hospice/palliative/home health preferred.
• Strong interpersonal and communication skills.
• Ability to work independently with flexible scheduling.
• Knowledge of hospice/palliative nutrition practices.

Responsabilidades

• Conduct comprehensive nutrition assessments and develop individualized care plans for patients in hospice and palliative care.
• Provide dietary counseling and education to patients, caregivers, and staff.
• Collaborate with interdisciplinary care team to align nutrition with overall patient care goals.
• Monitor patient progress and adjust nutrition recommendations as needed.
• Ensure compliance with regulations and best practices in medical nutrition therapy.
• Document patient interactions and care plans in electronic medical records in a timely manner.
• Stay current with evidence-based guidelines in hospice nutrition.

Requirements

• Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential required.
• Current licensure in the state of California.
• Minimum of two years of experience in clinical nutrition, preferably in hospice, palliative care, or home health settings.
• Strong interpersonal and communication skills to effectively engage with patients, caregivers, and healthcare teams.
• Ability to work independently and manage a flexible schedule based on patient needs.
• Knowledge of nutrition-related concerns in end-of-life care, including symptom management and palliative nutrition strategies.
