Job summary

The Center for Rehabilitation & Nursing at Washington Township is seeking a Part-time Dietitian to join our dedicated team. This role offers a 24-hour weekly schedule and competitive pay rates based on experience.

Responsibilities include assessing residents' nutritional needs, developing care plans, providing education to residents and families, and collaborating with the care team to ensure compliance with CMS and state regulations. Monday and Friday shifts with flexible scheduling.

Qualifications

• Registered by the Commission on Dietitetic and Registration.
• Hold a Certification / License in good standing within the state of practice.
• Knowledge of the state survey process and compliance with nutrition care regulations.
• Strong organization and communication skills.

Responsibilities

• Assess the clinical nutrition needs of residents/patients and develop a plan of care within the electronic medical record.
• Develop, Implement, and evaluate individualized care plans based on clinical findings, lab results, weight trends, intake and comorbidities.
• Provide nutrition education and counseling to resident, families and staff.
• Collaborate with interdisciplinary teams to develop a nutritional health plan for the patient's overall wellbeing
• Adhere to facility policy and procedures and to CMS, State and Federal regulations.
• Assist and participate interdisciplinary care conferences.
• Assist dietitian in duties as assigned.

Registered Dietitian

Pay Rate: $30.00-$35.00 per hour

Monday and Friday with Flexible Schedules
