Job summary

The Center for Rehabilitation & Nursing at Washington Township is seeking a Dietitian to join a dedicated team on the day shift. You will assess residents, develop and document nutrition plans in the EMR, and collaborate with clinicians to meet patient needs.

The position emphasizes individualized meal planning, ongoing nutritional updates, and adherence to policy. A Registered Dietitian with a Dietetics degree is required.

Qualifications

• Bachelor’s Degree in Dietetics from an accredited college or university.
• Registered Dietitian.
• Training in food management, diet therapy, etc.

Responsibilities

• Assess the clinical nutrition needs of residents/patients and develop and document the plan within the electronic medical record.
• Develop, implement, and evaluate individualized care plans based on clinical findings, lab results, weight trends, intake, and comorbidities
• Prepares individualized meal plans that consider the needs of the patient including; medical, nutritional, age, cultural, religious and ethical concerns
• Provides updates on the nutritional status of each patient weekly as part of team rounds/meetings and as requested
• Collaborates with medical professionals to assure patient nutrition needs are met.
• Completes all required documentation and reports in a timely manner, maintaining compliance with the policies and procedures
