Job summary

Personal Healthcare Management LLC is seeking a full-time Dietitian to cover nutrition care for residents at Plattsburgh and Massena facilities. You will assess, plan, and deliver nutrition services, including enteral care, and collaborate with nursing and food service teams.

Requirements include a bachelor’s degree (master’s in nutrition preferred), 1–3 years of direct care experience with 1–2 years in long-term care, and NY state dietetics/nutrition certification.

Qualifications

• Bachelor’s degree or higher from a regionally accredited college or university in the United States.
• Master’s degree with emphasis in nutrition preferred from an accredited program.
• One to three years direct care experience; 1–2 years in long-term care.
• Certification in dietetics-nutrition in New York State.

Responsibilities

• Assess nutrition status using lab data, fluid status, energy and protein needs.
• Evaluate data, review and modify plans of care, and deliver nutrition care.
• Provide nutrition information during meal rounds, test trays and menu checks.
• Plan and deliver nutrition care for residents fed by enteral means.
• Prepare residents for self-management of nutrition care.
• Support QA and quality improvement processes within the facility.
• Consult with Food Service Director to develop therapeutic diets.
• Refer to other health care providers as needed.
• Assist with department operations and related duties.
• Participate in inspections conducted by authorized agencies.
• Ensure safe resident nutrition care per physician/NP orders and standards.
• Maintain awareness of updates and apply them to practice.

Experience Required: One to three years’ direct care experience with one to two years’ experience in long- term care.

Professional Licensure and Certification Required: Certification in dietetics-nutrition in New York State

Essential Functions

1. Assess nutrition status, including laboratory data, fluid status, energy and protein needs ofresident.
2. Evaluates and interprets data, reviews and modifies plans of care, and delivers nutrition care to residents.
3. Routinely provides nutrition-related information/technical assistance by conducting meal rounds, test trays, menu checks and other services as needed.
4. Accurately assesses, plans and delivers nutrition care to residents fed by enteralmeans.
5. Prepare residents for self-management of nutritioncare.
6. Helps develop and implement Quality Assurance and Improvement processes within thefacility.
7. Consults with Food Service Director to develop appropriate therapeuticdiets.
8. Makes referrals as necessary to other health careproviders.
9. Completes other duties to support departmental operations as assigned orrequested.
10. Participates in inspections (surveys) conducted by authorized agencies
11. Provides safe resident nutritional care while demonstrating knowledge and competency in ordering nutritional interventions, and in accordance with physician/NP orders, according to established standards and policies.
12. Maintains awareness of new updates/standards and incorporates them into his/her nutritional practice throughout the continuum of care.
13. Assists co-workers as needed and maintains positive interpersonal relationships withcolleagues.
14. Exhibits professional conduct and behavior through cooperative interactions with others and accepts personal responsibility for their ownactions.
15. Reports any change in condition of a resident to the appropriate nursingsupervisor.
16. Reports any resident care issues to the appropriate nursing supervisor.
17. Assists co-workers as needed and maintains positive interpersonal relationships withcolleagues.
18. Exhibits professional conduct and behavior through cooperative interactions with others and accepts personal responsibility for their own actions.
