Job summary

Tarrytown Rehabilitation & Nursing Center in Village of Tarrytown, NY seeks a Dietitian/Nutrition Specialist for full-time, shared between two facilities. New rate up to $120k annually. Premium compensation and strong benefits package offered. Must be NY-certified; experience in long-term care preferred.

Responsibilities include nutrition assessment, care planning, enteral nutrition, and quality improvement initiatives, with collaboration across disciplines and ongoing professional development.

Qualifications

  • Bachelor’s degree or higher from a regionally accredited college or university in the United States (or an equivalent foreign degree)
  • Must be certified as a dietitian or nutrition professional in NY State once requirements met.
  • Master’s degree with emphasis in nutrition preferred.
  • One to three years’ direct care experience with one to two years’ experience in long- term care.
  • Certification in dietetics-nutrition in New York State, or successful completion of the Registered Dietitian examination or CNS exam acceptable.

Responsibilities

  • Assess nutrition status, including lab data, fluid status, energy and protein needs of residents.
  • Evaluates and interprets data, reviews and modifies plans of care, and delivers nutrition care.
  • Routinely provides nutrition-related information through meal rounds, menu checks and other services.
  • Plans and delivers nutrition care to residents fed by enteral means.
  • Prepare residents for self-management of nutrition care.
  • Assist QA/I processes within the facility.
  • Consult with Food Service Director to develop therapeutic diets.
  • Refer to other health care providers as needed.
  • Participate in inspections conducted by authorized agencies.
  • Maintain safety and compliance with physician/NP orders and established standards.

Essential Functions
  • Assess nutrition status, including laboratory data, fluid status, energy and protein needs ofresident.

  • Routinely provides nutrition-related information/technical assistance by conducting meal rounds, test trays, menu checks and other services as needed.
  • Accurately assesses, plans and delivers nutrition care to residents fed by enteralmeans.
  • Prepare residents for self-management of nutritioncare.
  • Helps develop and implement Quality Assurance and Improvement processes within thefacility.
  • Consults with Food Service Director to develop appropriate therapeuticdiets.
  • Makes referrals as necessary to other health careproviders.
  • Participates in inspections (surveys) conducted by authorized agencies
  • Provides safe resident nutritional care while demonstrating knowledge and competency in ordering nutritional interventions, and in accordance with physician/NP orders, according to established standards and policies.
  • Maintains awareness of new updates/standards and incorporates them into his/her nutritional practice throughout the continuum of care.
  • Assists co-workers as needed and maintains positive interpersonal relationships withcolleagues.
  • Exhibits professional conduct and behavior through cooperative interactions with others and accepts personal responsibility for their ownactions.
  • Reports any change in condition of a resident to the appropriate nursingsupervisor.
  • Reports any resident care issues to the appropriate nursing supervisor.
  • Assists co-workers as needed and maintains positive interpersonal relationships withcolleagues.
  • Exhibits professional conduct and behavior through cooperative interactions with others and accepts personal responsibility for their ownactions.
