Job summary

Houghton Rehabilitation and Nursing Center seeks a Registered Dietitian to assess nutrition status, develop plans of care, and provide nutrition care to residents in a long-term care setting.

The role involves conducting nutrition rounds, collaborating with the Food Service Director, and ensuring diet plans align with physician orders and standards. A NYS certification is required, with Master’s preferred.

Qualifications

• Bachelor’s degree or higher in nutrition or a related field; NYS dietitian certification required.
• Master’s degree with emphasis in nutrition preferred.
• 1–3 years of direct care experience in long-term care.

Responsibilities

• Assess nutrition status, including laboratory data, fluid status, energy and protein needs of residents.
• Evaluate and interpret data, review and modify plans of care, and deliver nutrition care to residents.
• Provide nutrition-related information and technical assistance via meal rounds, test trays, menu checks and related services.
• Plan and deliver nutrition care for residents fed by enteral means.
• Assist in self-management of nutrition care by residents.
• Develop and implement QA and improvement processes within the facility.
• Consult with Food Service Director to develop therapeutic diets.
• Make referrals to other healthcare providers as needed.
• Support departmental operations and participate in surveys by authorized agencies.

Educational Requirements

Experience Required

One to three years’ direct care experience with one to two years’ experience in long- term care.

Professional Licensure and Certification Required

Essential Functions

• Assess nutrition status, including laboratory data, fluid status, energy and protein needs ofresident.

• Routinely provides nutrition-related information/technical assistance by conducting meal rounds, test trays, menu checks and other services as needed.
• Accurately assesses, plans and delivers nutrition care to residents fed by enteralmeans.
• Prepare residents for self-management of nutritioncare.
• Helps develop and implement Quality Assurance and Improvement processes within thefacility.
• Consults with Food Service Director to develop appropriate therapeuticdiets.
• Makes referrals as necessary to other health careproviders.
• Completes other duties to support departmental operations as assigned orrequested.
• Participates in inspections (surveys) conducted by authorized agencies
• Provides safe resident nutritional care while demonstrating knowledge and competency in ordering nutritional interventions, and in accordance with physician/NP orders, according to established standards and policies.
• Maintains awareness of new updates/standards and incorporates them into his/her nutritional practice throughout the continuum of care.
• Assists co-workers as needed and maintains positive interpersonal relationships withcolleagues.
• Exhibits professional conduct and behavior through cooperative interactions with others and accepts personal responsibility for their ownactions.
• Reports any change in condition of a resident to the appropriate nursingsupervisor.
• Reports any resident care issues to the appropriate nursing supervisor.
• Assists co-workers as needed and maintains positive interpersonal relationships withcolleagues.
• Exhibits professional conduct and behavior through cooperative interactions with others and accepts personal responsibility for their ownactions.
