Job summary

Personal Healthcare Management LLC is seeking a part-time Dietitian/Nutritionist to work across two facilities in New York. You will assess residents' nutrition status, develop care plans, and provide education and nutrition services in coordination with the care team.

Bachelor’s degree required; NY licensure as a dietitian/nutritionist. 1–3 years direct care in long-term care preferred; Master’s degree in nutrition is a plus.

Qualifications

• Bachelor’s degree required; NY licensure as dietitian/nutritionist.
• Master’s degree in nutrition preferred.
• 1–3 years direct care experience in long-term care.
• Certification in NY State upon meeting requirements.

Responsibilities

• Assess nutrition status and needs of residents.
• Evaluate and modify care plans; deliver nutrition care.
• Provide nutrition education via rounds, menus, and services.
• Plan and deliver nutrition care for residents receiving enteral feeding.
• Support QA and improvement processes within the facility.
• Consult with Food Service Director on therapeutic diets.
• Refer to other health care providers as needed.
• Perform other duties to support department operations.

Qualifications: 1) Bachelor’s degree or higher from a regionally accredited college or university in the United States. Must be certified as a dietitian or nutrition professional in NY State once requirements met *See Professional Licensure and Certification Requirements

Educational Requirements

Bachelor’s degree required; Master’s degree with emphasis in nutrition preferred from an accredited program.

Experience Required

One to three years’ direct care experience with one to two years’ experience in long- term care.

Professional Licensure and Certification Required

Certification in dietetics-nutrition in New York State

Essential Functions

• Assess nutrition status, including laboratory data, fluid status, energy and protein needs ofresident.
• Evaluates and interprets data, reviews and modifies plans of care, and delivers nutrition care to residents.
• Routinely provides nutrition-related information/technical assistance by conducting meal rounds, test trays, menu checks and other services as needed.
• Accurately assesses, plans and delivers nutrition care to residents fed by enteralmeans.
• Prepare residents for self-management of nutritioncare.
• Helps develop and implement Quality Assurance and Improvement processes within thefacility.
• Consults with Food Service Director to develop appropriate therapeuticdiets.
• Makes referrals as necessary to other health careproviders.
• Completes other duties to support departmental operations as assigned orrequested.
• Participates in inspections (surveys) conducted by authorized agencies
• Provides safe resident nutritional care while demonstrating knowledge and competency in ordering nutritional interventions, and in accordance with physician/NP orders, according to established standards and policies.
• Maintains awareness of new updates/standards and incorporates them into his/her nutritional practice throughout the continuum of care.
• Assists co-workers as needed and maintains positive interpersonal relationships withcolleagues.
• Exhibits professional conduct and behavior through cooperative interactions with others and accepts personal responsibility for their ownactions.
• Reports any change in condition of a resident to the appropriate nursingsupervisor.
• Reports any resident care issues to the appropriate nursing supervisor.
• Assists co-workers as needed and maintains positive interpersonal relationships withcolleagues.
• Exhibits professional conduct and behavior through cooperative interactions with others and accepts personal responsibility for their ownactions.
