Job summary

Millennium-Post-Acute is seeking a registered dietitian to lead our nutrition team across two sister facilities in West Columbia, SC. This role emphasizes collaborative care with nursing, therapies, and social services to tailor dietary plans and improve patient outcomes.

Qualifications

• Registered dietitian licensure or credentialing is expected.
• Ability to coordinate with multiple therapy teams and nursing staff.

Responsibilities

• Advises and consults with Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, and Socal Services to provide appropriate dietary services.
• Assess patient/resident medications for food and/or drug interactions.
• Interpret patient/resident laboratory results in order to individualize dietary guidelines.
• Provides diet counseling service in accordance with the plan of care as ordered by the patient/resident's physician, and approved by the IDT.
