Centers Dialysis Care is seeking Registered Dialysis Nurses (RN) for our home hemodialysis program in a skilled nursing facility located in Bronx, NY.

Duties

• Collaborate with the interdisciplinary team to assess patient status and implement and monitor care progression
• Perform care coordination and patient education
• Administer medications and treatments as directed by providers/physicians
• Monitor patients throughout treatment and document responses
• Mobilize resources in complex cases to maximize patient participation in recovery
• Integrate assessment and diagnostic information to anticipate healthcare needs
• Ensure effective handoff communication and reporting
• Adjust staffing patterns as needed based on unit needs
• Provide individualized comfort measures using evidence-based practices
• Participate in performance improvement activities
• Perform other related duties as required

Requirements

• Active NY RN License
• Dialysis experience required
• Nx Stage machine experience a plus
• CPR Certification
• Clinical nursing experience in dialysis preferred

Rate: $45.00 - $55.00
