We are hiring for the Telehealth Nurse Practitioner position at Vis-À-Vis Health. This role involves providing remote patient care via telehealth platforms, focusing on transitional and chronic care management for geriatric patients.

Salary Range

$75,000.00/yr - $140,000.00/yr (based on skills and experience)

Requirements

• Must have 4 or more active NP licenses in PA, NY, NJ, FL, CT, MA, NH, NM, VT, TX, WV
• Current and valid RN and NP licenses
• Minimum 3 years of clinical nursing experience in relevant settings
• Experience with Medicare and Medicaid billing
• Proficiency in electronic medical records and Microsoft Office
• Excellent communication and interpersonal skills; bilingual in English/Spanish preferred

Responsibilities

• Conduct telehealth video visits with patients
• Review medical histories and formulate diagnoses
• Manage medication prescriptions and patient treatment plans
• Collaborate with case managers and participate in team meetings
• Ensure HIPAA compliance and maintain patient confidentiality
