CareBridge Nurse Practitioner (Bilingual)

This role enables associates to work virtually full-time, providing maximum flexibility and autonomy. This approach promotes productivity, supports work-life integration, and ensures essential face-to-face onboarding and skill development.

Role Overview:

The CareBridge Nurse Practitioner (Bilingual) is responsible for collaborating with physicians, other healthcare providers, and family members to develop complex care plans aligned with patients' health goals and values. Provides clinical and non-clinical support to patients.

How You Will Make an Impact

Primary duties include, but are not limited to:

  1. Providing primary and urgent care via telephone and tele-video to patients receiving home and community-based services through Medicaid, dual eligible members, and other assigned members.
  2. Developing and implementing care plans for adults with chronic and complex conditions.
  3. Gathering patient history, conducting physical exams, diagnostics, and developing treatment plans based on patient goals and conditions.
  4. Identifying and closing gaps in care.
  5. Supporting patients and families physically and psychosocially with input from the interdisciplinary team.
  6. Educating patients and families on medication use, side effects, illness progression, diet, adherence, and crisis prevention.
  7. Coordinating with clinical team members and other healthcare providers to ensure timely and appropriate care.
  8. Maintaining accurate medical records and documentation per regulations and standards.
  9. Participating in ongoing education as required.
  10. Prescribing medication within state authority.
Minimum Requirements

MS in Nursing, active national NP certification, valid Family or Adult Nurse Practitioner license in applicable states, EMR experience, and bilingual skills. Experience with complex care cases for developmental disabilities and chronic illness is strongly preferred.

Compensation

For candidates in Jersey City, NJ, the salary range is $129,216 to $193,824. Benefits include comprehensive packages, incentive programs, stock purchase, and 401k. Compensation varies based on experience, education, location, and market factors.
