Job summary

U.S. Renal Care is seeking a Licensed Master Social Worker to provide social services to patients and their families, aiming to promote maximum social functioning and psychological adjustment to dialysis and rehabilitation.

The role includes coordinating care with the interdisciplinary team, conducting initial psychosocial assessments, and supporting patient outcomes within CMS and state regulations. Strong collaboration with medical staff is expected.

Responsibilities

  • Assist with clinical and operational processes to improve patient health and minimize hospitalizations.
  • Coordinate transient arrangements for patients.
  • Demonstrate effective use of supplies.
  • Perform duties to meet patient care or operational needs.
  • Identify and counsel psychosocial issues.
  • Provide patient and family education.
  • Support achievement of program target goals for patient outcomes.
  • Provide information and referrals as needed.
  • Attend monthly QAPI activities.
  • Coordinate development and maintenance of patient/family support groups.
  • Maintain collaborative working relationship with Medical Director and physicians.
  • Establish and maintain positive relationships with area hospitals, agencies, vendors and the community.
  • Actively promote GUEST customer service standards; develop effective relationships at all levels of the organization.
  • Attend staff meetings and continuing education offerings.
  • Educate staff and provide inservice training in relation to patient psychosocial care.
  • Lead staff in team concepts and promote a team effort.
