Fresenius Medical Care seeks a Social Worker to deliver psychosocial services to dialysis patients and families. You will assess, plan, educate and counsel within an interdisciplinary team to promote rehabilitation, adjustment and quality of life.

The role supports advance directives, transplant referrals and collaboration with insurance and financial assistance teams to optimize care and adherence. Some travel may be required.

Qualifications

• Masters in Social Work and state licensure required.
• 2–5 years’ related experience in psychosocial services or dialysis settings preferred.
• Ability to provide assessment, counseling and education to patients and families.

Responsabilités

• Assess psychosocial status and rehabilitation-related needs.
• Participate in care planning with patient and healthcare team.
• Provide counseling to address adherence, quality of life and transplant referrals.
• Educate patients and families about treatment options and resources.
• Coordinate with Insurance Coordinators on billing and coverage issues.
• Support staff with psychosocial topics and participate in team huddles.
