Job summary

U.S. Renal Care is seeking a Licensed Master Social Worker to provide psychosocial support to dialysis patients and families, promoting adjustment and adherence, and collaborating with the care team.

Responsibilities include assessments, coordinating resources, education, and participating in quality improvement activities. A CSWE-accredited MSW and state licensure are required; LCSW preferred. Strong computer skills and experience with healthcare systems are expected.

Qualifications

• Must have Master's Degree in Social Work accredited by CSWE.
• Current licensure in applicable state is required unless listed otherwise.
• Experience providing social services to dialysis patients preferred.
• Proficient in spoken and written English.
• Basic computer skills including Microsoft Office; proficiency in USRC clinical applications within 90 days of hire.

Responsibilities

• Provide social services to dialysis patients and families to promote social functioning and adjustment.
• Assist with clinical and operational processes to improve patient health and minimize hospitalizations.
• Identify social agencies and resources for patient needs including financial/funding sources.
• Complete initial assessment of new patient psychosocial needs under state and CMS regulations.
• Participate as a member of the Interdisciplinary team in patient assessment and care planning.
