U.S. Renal Care seeks a Licensed Master Social Worker to provide social services for dialysis patients and families, promoting psychological adjustment and social functioning through timely support.

The role includes psychosocial assessment, education for patients and families, and collaboration with the care team to meet program goals while complying with state and federal regulations.

Competenze

• Master's Degree in Social Work (CSWE) required; LCSW preferred.
• Current licensure in the applicable state required unless employed in AZ/PA/Guam; must be current and in good standing.
• Experience providing social services to dialysis patients preferred.
• Excellent verbal and written communication skills in English.
• Proficiency in Microsoft Office and USRC clinical applications within 90 days of hire.

Mansioni

• Provide social services to patients and families to promote psychosocial functioning and adjustment to dialysis.
• Identify psychosocial issues and counsel patients and families.
• Coordinate with Interdisciplinary team for patient assessments and care planning.
• Develop connections with social agencies and resources for patient needs.
• Attend Quality Assessment and Performance Improvement (QAPI) activities.
