U.S. Renal Care, Inc. is seeking a Licensed Master Social Worker to provide timely psychosocial services to dialysis patients and families, aiding social functioning and psychological adjustment to treatment.

The role supports clinical and operational processes to improve patient outcomes and reduce hospitalizations. The candidate should hold a CSWE-accredited MSW, with state licensure (LCSW preferred) and experience in dialysis-related social work.

Qualifications

• Must have Master’s Degree in Social Work (CSWE-accredited).
• Current licensure in applicable state is required unless in AZ, PA or Guam.
• Previous experience providing social services to dialysis patients preferred.
• Proficiency in English communication, both verbal and written.
• Basic computer skills including MS Office; proficiency in USRC clinical applications within 90 days.

Responsabilités

• Provide social services to patients and families to promote psychosocial functioning.
• Assist with clinical and operational processes to improve patient health and reduce hospitalizations.
• Identify social agencies and resources for patient needs, including funding options.
• Participate as a member of the Interdisciplinary team in patient assessment and care planning.
• Attend staff meetings and contribute to staff development and education.
