Job summary

U.S. Renal Care is seeking a Social Worker to join an interdisciplinary team focused on helping patients with kidney disease adjust to dialysis and rehabilitation. You will assess psychosocial needs, provide education, and connect patients to social services and community resources.

Responsibilities include coordinating care plans, advocating for patients, and collaborating with Medical Directors, hospitals, and agencies to improve outcomes and reduce hospitalizations.

Qualifications

• As part of the team, assess psychosocial needs and complete the KDQOL per policy and regulations.
• Identify psychosocial issues and provide patient/family education.
• Coordinate support communities, resources, and referrals for patients and families.
• Represent patients with local, state, and federal agencies as needed.

Responsibilities

• Conduct patient psychosocial assessments and care planning activities.
• Counsel patients and families on social needs and available resources.
• Collaborate with the Medical Director, physicians, and community partners.
• Participate in required ongoing education and QAPI activities.
