Health Here seeks a dedicated Social Worker to provide psychosocial services to dialysis patients, supporting emotional well-being and treatment adherence. You will assess needs, develop care plans, and offer counseling in collaboration with the healthcare team.

You will educate patients on kidney disease, treatment options, advance directives, and coordinate discharge planning, insurance navigation, and community referrals as needed.

Qualifications

• Master's in Social Work with state licensure.
• Strong assessment, counseling, and communication skills, with the ability to collaborate effectively within multidisciplinary teams.
• Physical ability to support travel and home visits.
• No supervision required.

Responsibilities

• Provide psychosocial services to dialysis patients to support emotional well-being and behavioral health.
• Assess patients' psychosocial status and develop individualized care plans.
• Offer counseling to promote treatment adherence, quality of life, and transplant referral.
• Facilitate patient education on kidney disease, treatment options, and advance directives.
• Assist with discharge planning, insurance navigation, and referrals to community resources.
• Support quality improvement initiatives and adhere to organizational policies.
• Contribute to team discussions and staff training as needed.
