Job summary

US Renal Care seeks a dedicated Social Worker to support patients living with kidney disease, helping them adjust to dialysis and rehabilitation while coordinating psychosocial care within an interdisciplinary team.

You will assess psychosocial needs, counsel patients and families, and educate on resources and referrals to optimize overall care and quality of life within regulatory compliance.

Qualifications

• Experience in psychosocial care for dialysis patients.
• Knowledge of KDQOL and CMS/state regulations.
• Strong communication and empathetic patient advocacy.

Responsibilities

• Conduct patient psychosocial assessments and care planning in line with policy.
• Counsel patients and families; provide education on resources and funding.
• Collaborate with Medical Director, physicians, and community partners to improve patient outcomes.
