Job summary

US Renal Care is seeking a Social Worker to join its interdisciplinary team, helping dialysis patients achieve better psychosocial functioning and adjustment to treatment. You will perform patient assessments, develop care plans, and provide education and referrals.

The role requires a CSWE-accredited MSW, active licensure in the relevant state, strong English communication, and proficiency with MS Office and USRC applications; you will participate in QAPI activities and collaborate with

Qualifications

  • Master's Degree in Social Work accredited by CSWE.
  • Current licensure in good standing in the applicable state (except AZ/PA/Guam eligibility).
  • Demonstrated working knowledge of English; able to communicate verbally and in writing.
  • Basic computer skills, including Word, Excel and Outlook.
  • Proficiency in USRC clinical applications within 90 days of hire.

Responsibilities

  • Conduct psychosocial assessments and care planning per policy and regulations.
  • Identify and counsel psychosocial issues; provide patient and family education.
  • Coordinate community resources, social agencies, and referrals.
  • Collaborate with interdisciplinary team; participate in training and meetings.
  • Support QAPI activities and ensure compliance with laws and regulations.
