Job summary

Renalliance Group is seeking a highly motivated Renal Social Worker for a state-of-the-art dialysis center in New York. The role requires current LMSW or LCSW licensure and involves psychosocial assessment, patient support, counseling, and patient education within an interdisciplinary team.

The position includes developing care plans, coordinating facility transfers, and contributing to ESRD Network/CrownWeb reporting.

Qualifications

• Must hold LMSW or LCSW license.
• Experience in clinical settings preferred.
• Experience in dialysis or ESRD settings a plus.

Responsibilities

• Conduct psychosocial assessments of patients.
• Provide patient support and individual counseling.
• Educate patients and families about treatment options and care plans.
