Qualifications

• Master's Degree in Social Work accredited by CSWE.
• LCSW licensure preferred where applicable.
• Experience providing social services to dialysis patients preferred.
• Strong English communication skills, both written and verbal.
• Proficiency with standard computer software (Microsoft Office).

Responsibilities

• Provide social services to patients and families to promote optimal psychosocial adjustment.
• Assist with clinical and operational processes to improve patient health and reduce hospitalizations.
• Identify social agencies and resources to support patient needs and funding.
• Participate as a member of the interdisciplinary team in care planning.
