Qualifications

• MSW required and licensed to practice in the state.
• Clinical social work skills for dialysis patients.
• Ability to coordinate services and resources.
• Experience with patient advocacy and education.

Responsibilities

• Provide patient support addressing emotional, financial and lifestyle changes due to kidney disease.
• Assist patients in utilizing community resources for nutrition, transport and housing.
• Advocate for patients to access healthcare and benefits.
• Educate patients and families on ESRD treatment options and dialysis modalities.
• Coordinate translation services for patients and caregivers as needed.
• Develop individualized care plans with the interdisciplinary team.
• Support care planning and modification based on psychosocial assessment.
• Act as ESRD Network liaison and participate in quality improvement.
