Qualifications

• One (1) year of work experience in a behavioral or healthcare-related setting, preferred.
• Master's Degree in Social Work from an accredited CSWE institution.
• MSW required. LCSW/ACSW required, LCSW preferred (CA). Licensure/LMSW required in states with licensure (TX, TN).

Responsibilities

• Educate patients on treatment, health, and wellness to enhance quality of life for those living with kidney disease.
• Conduct psychosocial assessments and participate in care planning and team reviews of psychosocial needs.
• Develop and manage services to help patients cope with chronic kidney disease and dialysis-related challenges.
