Avera is seeking a Registered Nurse to provide major portions of self-care and home dialysis training and monitoring for dialysis patients and/or their caregivers. The role involves educating patients, coordinating with the medical team, and ensuring adherence to regulatory standards.

You will conduct monthly patient assessments, document thoroughly, and support patients in managing home dialysis therapy, with on-call responsibilities and a possible Monday–Friday schedule and occasional nights,

Formación

• Active RN license in the state of practice.
• 1-3 years nursing experience.
• Ability to communicate effectively with patients and families.
• Visual acuity adequate to perform position duties.

Responsabilidades

• Assesses patient pre-therapy to determine if patient meets selection criteria.
• Develops and implements education plans for patients and caregivers on dialysis processes and home therapy.
• Monitors patients monthly to problem-solve and mentor safe delivery of home therapy.
• Coordinate care with Medical Director/Nephrologist, patients, families, and other staff.
• Maintain accurate documentation and electronic records according to policy.
• Promote compliance with CMS/ESRD-rRNA standards and safety protocols.
• Facilitate patient supply re-orders and related tasks.
