Centers Dialysis Care is seeking Registered Dialysis Nurses (RN) for our home hemodialysis program in a skilled nursing facility located in Bronx, NY.

Duties

• Assess patient status and implement and monitor care in collaboration with the interdisciplinary team
• Coordinate care and educate patients
• Administer medications and treatments as directed by healthcare providers
• Monitor patients during treatment and document responses
• Mobilize resources in complex cases to enhance patient participation in recovery
• Integrate assessment and diagnostic information to anticipate healthcare needs
• Ensure effective handoff communication and report on potential issues
• Manage staffing patterns proactively based on unit needs
• Provide individualized comfort measures using evidence-based and innovative approaches
• Participate in unit performance improvement activities
• Perform other related duties as required

Requirements:

• Active NY RN License
• Dialysis experience required
• Nx Stage machine experience a plus
• CPR Certification
• Clinical nursing experience in dialysis preferred

Rate: $45.00 - $55.00 per hour
