Centers Dialysis Care - Registered Dialysis Nurse (RN)

Centers Dialysis Care is seeking experienced Registered Dialysis Nurses (RN) for our home hemodialysis program at a skilled nursing facility in Bronx, NY. We are a high-level network of dialysis facilities across Brooklyn, Bronx, and Rockland County, NY. We welcome both experienced nurses and those interested in training in dialysis care.

Responsibilities

• Assess patient status and monitor care in collaboration with the interdisciplinary team.
• Coordinate care and educate patients.
• Administer medications and treatments as directed.
• Monitor and document patient responses to treatments.
• Mobilize resources for complex cases to maximize patient participation in recovery.
• Use assessment and diagnostic information to anticipate healthcare needs.
• Ensure effective handoff communication and report on potential issues.
• Adjust staffing and develop individualized comfort measures based on patient needs.
• Participate in performance improvement activities.
• Perform other duties as assigned.

Requirements

• Active NY RN License
• Dialysis experience required
• Nx Stage machine experience a plus
• CPR Certification

Rate: $45.00 - $55.00 per hour
