Responsibilities

• Welcome assigned patients and inquire about their wellbeing since their last treatment.
• Evaluate vascular access for patency and monitor cannulation and heparin administration as delegated.
• Record pre and post treatment vital signs and weight and document vascular access evaluation.
• Initiate dialysis treatment according to prescribed orders including QB and QD.
• Evaluate intradialytic problems and provide intervention as prescribed or directed.
• Monitor patient responses to dialysis therapy and document changes.
• Record and document pre and post dialysis evaluation, weight and vital signs with initial ID.
• Calculate and enter dialysis machine programming for UF goal, treatment time, and UF modeling.
• Ensure safety alarms are enabled and documented.
• Maintain Hemodialysis Treatment Sheet and progress notes with accurate documentation.
• Report significant changes in patient condition to the RN or supervisor.
• Observe patient and perform machine safety checks; report unusual findings.
• Document interventions for unusual patient status and patient responses.
• Evaluate vascular access during treatment, monitor pressures, and report findings.
• Obtain prescribed labs and prepare specimens for collection.
• Ensure all blood spills are cleaned per policy and organize supplies to prevent waste.
