Job summary

You will assess, plan, implement, and evaluate care for dialysis patients, coordinating with the care team. Responsibilities include medication administration, documentation, and ongoing patient education.

Qualifications

• 12 months or more of recent dialysis experience required.
• Must have RN license and meet state practice requirements.
• Floating to area sites within 50 miles of main site.

Responsibilities

• Assess, document, and adjust patient care plans during hemodialysis.
• Administer medications as prescribed and monitor effectiveness.
• Perform data collection and analysis pre-during-post dialysis; notify supervisor as needed.
• Collaborate with Interdisciplinary Team for education and care plan updates.
• Direct and support daily delivery of safe dialysis therapy to patients.
